When the U.S. Spends More on Fighting a Disease, the Rest of the World Spends Less

Professor David Ridley studies government funding of disease treatments

When the U.S. spends more to research cures for a neglected disease, other countries spend less on that same disease, according to new research from Duke University’s Fuqua School of Business.

Professor David Ridley found a 10 percent increase in U.S. funding was associated with a 2-3 percent reduction by other governments. It is the first time the effects of changes in U.S. research funding on other countries has been measured.

“Governments might be free riding on one another,” Ridley said. “Alternatively, governments might be shifting resources away from diseases heavily funded by the U.S. to diseases receiving relatively little funding by the U.S. Either way, we need to pay attention because these diseases inflict a horrible global burden.”

A wide range of tropical diseases sicken and kill thousands of people worldwide each year. They’re considered neglected because treatments aren’t profitable, since most sufferers are in impoverished areas.

“Funding research into tropical disease treatments is one of the best ways the U.S. can improve global well-being,” Ridley said.

Ridley’s previous research includes a proposal for a federal voucher system to encourage private firms to develop tropical disease treatments. Congress made that proposal law in 2007.

“There are many cases in which all countries increase funding, for high profile diseases or after headline-grabbing outbreaks.”

The latest research, Strategic interaction among governments in the provision of a global public good, is published in the Journal of Public Economics. Ridley worked with Margaret Kyle of Mines ParisTech and PSL Research University and Duke Ph.D. student Su Zhang.

The researchers studied data collected annually by the non-profit Policy Cures that accounts for every dollar spent on neglected disease research and development since 2007.

There are many cases in which all countries increase funding, for high profile diseases or after headline-grabbing outbreaks.

“If you do a simple correlation between U.S. and international funding, it will be positive because governments share an interest in funding diseases with high burden,” Ridley said. “But if you only look at that, then you’re missing something.”

The researchers looked for factors beyond the disease itself to isolate how changes in U.S. government funding — which accounted for more than half of global research funding for 15 neglected diseases from 2007 to 2014 — affected funding from governments and foundations in 41 other countries.

“We needed an effect unrelated to the disease, something that shocks U.S. funding,” Ridley said.

“If the U.S. government is heavily funding one disease, then other governments can focus on other diseases.”

The researchers used changes in the membership of congressional appropriations committees. Senators or representatives who come from states with major research universities tend to seek more funding for the National Institutes of Health, which supports those schools.

“If the appropriations committees have more members of Congress from states representing major research universities, then there’s more funding for medical research, and we see other countries adjust to that,” Ridley said. “Conversely, there is less N.I.H. funding when there are fewer such committee members, and other countries increase funding.”

Ridley said some of the funding change might be good for global health. “If the U.S. government is heavily funding one disease, then other governments can focus on other diseases.”

The U.S. spends $840 million on HIV/AIDS research each year. The impact of the disease in Brazil is almost twice what it is in the U.S.,  but the Brazilian government spends less than a million dollars per year on HIV research.

In contrast, Brazil spends $7 million a year — more than it spends on any other disease — on research for dengue, which has much less impact on its population than HIV/AIDS.. There are no known treatments for dengue, also known as breakbone fever, a mosquito-borne viral infection that can be fatal. While cases are underreported, the World Health Organization estimates dengue affects about 96 million people each year.

“Dengue might receive that funding in Brazil because the U.S. is spending so much on research into HIV/AIDS,” Ridley said. “HIV is of course a huge burden, so maybe every funder out there looks at it. But it might be good if Brazil focuses on dengue rather than HIV.”

Besides spending far more in magnitude than other countries, the U.S. also spends more on disease research as a share of GDP than do other countries.

“The U.S. is fairly criticized in some ways,” Ridley said, “but it deserves credit for what it’s doing in neglected diseases, and other countries should be encouraged to do more — especially when you consider the awful impact of the diseases.”

Awareness of the past generosity leads to feelings of stewardship for future others.

Legacy motive can break a harmful cycle of negative intergenerational reciprocity.

Abstract

Sometimes the well-intended actions of prior generations result in undesirable outcomes to subsequent generations. Our research highlights the role of making the intention of past generations transparent in inter-generational resource allocations.

We demonstrate that even in the presence of undesirable outcomes, people use previous generations’ intentions to guide their own decisions for allocating resources to future generations.

Furthermore, we show that making the good intention of past generations transparent in inter-generational resource allocations enacts feelings of stewardship, which in turn promotes generosity to future others.

Finally, we identify an intervention – the induction of legacy motivations – that can prevent a pattern of selfish intentions from being reciprocated forward in time to future generations.

Psychopathy Linked to Specific Structural Abnormalities in the Brain

New research provides the strongest evidence to date that psychopathy is linked to specific structural abnormalities in the brain.

The study, published in Archives of General Psychiatryand led by researchers at King’s College London is the first to confirm that psychopathy is a distinct neuro-developmental sub-group of anti-social personality disorder (ASPD).

Most violent crimes are committed by a small group of persistent male offenders with ASPD. Approximately half of male prisoners in England and Wales will meet diagnostic criteria for ASPD. The majority of such men are not true psychopaths (ASPD-P). They are characterised by emotional instability, impulsivity and high levels of mood and anxiety disorders. They typically use aggression in a reactive way in response to a perceived threat or sense of frustration.

However, about one third of such men will meet additional diagnostic criteria for psychopathy (ASPD+P). They are characterised by a lack of empathy and remorse, and use aggression in a planned way to secure what they want (status, money etc.). Previous research has shown that psychopaths’ brains differ structurally from healthy brains, but until now, none have examined these differences within a population of violent offenders with ASPD.

Dr Nigel Blackwood from the Institute of Psychiatry at King’s and lead author of the study says: ‘Using MRI scans we found that psychopaths had structural brain abnormalities in key areas of their ‘social brains’ compared to those who just had ASPD. This adds to behavioural and developmental evidence that psychopathy is an important subgroup of ASPD with a different neurobiological basis and different treatment needs.

‘There is a clear behavioural difference amongst those diagnosed with ASPD depending on whether or not they also have psychopathy. We describe those without psychopathy as ‘hot-headed’ and those with psychopathy as ‘cold-hearted’. The ‘cold-hearted’ psychopathic group begin offending earlier, engage in a broader range and greater density of offending behaviours, and respond less well to treatment programmes in adulthood, compared to the ‘hot-headed’ group. We now know that this behavioural difference corresponds to very specific structural brain abnormalities which underpin psychopathic behaviour, such as profound deficits in empathising with the distress of others.’

The researchers used Magnetic Resonance Imaging (MRI) to scan the brains of 44 violent adult male offenders diagnosed with Anti-Social Personality Disorder (ASPD). Crimes committed included murder, rape, attempted murder and grievous bodily harm. Of these, 17 met the diagnosis for psychopathy (ASPD+P) and 27 did not (ASPD-P). They also scanned the brains of 22 healthy non-offenders.

The study found that ASPD+P offenders displayed significantly reduced grey matter volumes in the anterior rostral prefrontal cortex and temporal poles compared to ASPD-P offenders and healthy non-offenders. These areas are important in understanding other people’s emotions and intentions and are activated when people think about moral behaviour. Damage to these areas is associated with impaired empathising with other people, poor response to fear and distress and a lack of ‘self-conscious’ emotions such as guilt or embarrassment.

Dr Blackwood explains: ‘Identifying and diagnosing this sub-group of violent offenders with brain scans has important implications for treatment. Those without the syndrome of psychopathy, and the associated structural brain damage, will benefit from cognitive and behavioural treatments. Optimal treatment for the group of psychopaths is much less clear at this stage.’

Pricey personal loans would be outlawed by California bill

California lawmakers are once again trying to cap the interest rate that lenders can charge on large personal loans, renewing an effort to eliminate the state’s flourishing market for super-expensive debt. A bill introduced Thursday by Assemblyman Ash Kalra (D-San Jose) could dramatically reshape California’s lending industry by capping interest rates at roughly 20% for consumer loans between $2,500 and $10,000. Since rate caps were removed by the Legislature in the 1980s, there’s been no limit to the amount of interest lenders can charge on those loans.(Los Angeles Times)

Google tests system to help locate 911 callers

Google quietly ran a test of new technology to make it easier for 911 operators to locate cellphone callers, and 911 centers that participated said the results were promising. The nation’s existing 911 system, which turns 50 this month, has struggled with the explosion of cellphones. The vast majority of 911 calls these days are made using a cellphone, but the location of the caller is hard to pinpoint.(Wall Street Journal)

Uber CEO predicts Amazon-style losses to fuel lofty goals

Speaking at a conference, Uber CEO Dara Khosrowshahi rattled through the company’s ambitions: food delivery, freight, autonomous vehicles and even buses and bikes. Ultimately, Uber Technologies Inc.’s business is getting people from “point A to point B,” he said at a Goldman Sachs-hosted event this week. Achieving such goals means the company will continue to lose money, he acknowledged, a day after Uber reported a loss of $4.5 billion for 2017 on $7.5 billion in net revenue.(Automotive News)

Probe finds bitcoin mining interfered with broadband network

The Federal Communications Commission said on Thursday an investigation had found that a Brooklyn bitcoin mining operation interfered with T-Mobile U.S. Inc.’s broadband network. The company had complained about interference to its 700 MHz LTE network in Brooklyn from radio emissions it said were coming from a Brooklyn residence mining for the cryptocurrency – verifying bitcoin transactions.(Reuters)

US digital banking startup Azlo readies for launch

Azlo, a U.S. digital banking startup that is majority-owned by Banco Bilbao Vizcaya Argentaria S.A., will open for business this week, marking the latest effort by the Spanish lender to attract a new generation of digital-savvy customers. The startup, which will offer online banking services to small businesses and freelancers, was built within a BBVA division that funds stand-alone fintech startups, the companies said.(Reuters)

Senators press IRS on community benefit standards

Leading GOP senators are pressing the IRS on whether not-for-profit hospitals are meeting the standards of charity care and other community benefits required by their tax-exempt status, and how the agency enforces current policies if they’re not. In a letter to David Kautter, acting commissioner of the IRS, Senate Finance Committee Chair Orrin Hatch, R-Utah, and Judiciary Committee Chair Charles Grassley, R-Iowa, demanded to know whether the agency collects information on not-for-profits’ contributions to their communities.(Modern Healthcare)

Salon is likely the first publisher to ask users running ad blockers to either turn them off or make up for it by letting Salon use their computers’ processing power to mine cryptocurrency in the background. McDonald’s makes Happy Meals (slightly) healthier

McDonald’s has fresh goals to improve the food it markets to kids, an effort that appears lofty to some and lame to others. The plan is being touted by the Golden Arches as “an expanded commitment to families.” It comes with the backing of the Alliance for a Healthier Generation, a group formed by the American Heart Association and the Clinton Foundation that McDonald’s began working with in 2013, when it took soda off the Happy Meal menu board.(Ad Age)

When we are in alignment with our gifts

I want to be in alignment with my gifts to transform my health and health of others by writing a book. My goal is to use my experience in healing my 82 year old mother to active life and share my love of wellness and healing with others. As a family health coach, I want to lead the whole family to wellness in the bay area. In doing so, I can be full time helping others and also transform my health and the people I touch. I can finish this book in 9 weeks. The time is now.

I will start today with an online training program for all my viewers. You will receive an early copy of my ebook – family health transformation or title To Be Identified.
