According to the Centers for Disease Control and Prevention (CDC), the 2017-2018 flu season appears particularly virulent. Influenza activity increased significantly in December, with influenza A(H3N2) viruses predominating, and in a December 27 notice, the agency noted that “In the past, A(H3N2) virus-predominant influenza seasons have been associated with more hospitalizations and deaths in persons aged 65 years and older and young children …”1

While influenza can indeed be deadly in rare cases, what most health experts fail to tell you is that these deaths are typically the result of secondary infections, not the flu virus itself. Importantly, research has highlighted the link between influenza and severe sepsis — a progressive disease process initiated by an aggressive, dysfunctional immune response to an infection in the bloodstream (which is why it’s sometimes referred to as blood poisoning).

Symptoms of sepsis are often overlooked, even by health professionals, and without prompt treatment, the condition can be deadly. Unfortunately, conventional treatments often fail, and most hospitals have yet to embrace the use of intravenous (IV) vitamin C, hydrocortisone and thiamine,2 which has been shown to reduce sepsis mortality from 40 to a mere 8.5 percent.3,4

How Influenza May Cause Lethal Sepsis

According to researchers, “Severe sepsis is traditionally associated with bacterial diseases … However, viruses are becoming a growing cause of severe sepsis worldwide.” As noted in the video above, some sepsis symptoms also resemble influenza, which can lead to tragedy. Millions of families get their taxes done every year for free through two programs sponsored by the IRS. These help people with lower and moderate incomes:

  • The Volunteer Income Tax Assistance: This program is also known as VITA. It offers free tax return preparation to eligible taxpayers who generally earn $54,000 or less.
  • Tax Counseling for the Elderly: TCE is mainly for people age 60 or older, but offers service to all taxpayers. The program focuses on tax issues unique to seniors. AARP participates in the TCE program through AARP Tax-Aide.

The IRS works with local community groups to train and certify thousands of volunteers. Eligible taxpayers should consider taking advantage of these free programs. This includes people with disabilities and people who speak limited English.

Here are some additional details about these two volunteer programs:

  • Free Tax Prep Around the Country. The IRS works with community organizations to offer free tax help at thousands of sites nationwide. These sites usually begin opening in late January and early February.
  • Free Electronic Filing. VITA and TCE provide free electronic filing. E-file is the safest, most accurate way to file a tax return. Taxpayers can combine e-file with direct deposit for quicker refunds.
  • Volunteer Preparers Trained to Help Find Tax Benefits. The IRS certifies the VITA and TCE volunteers. They help people get all the tax benefits for which they are eligible. These include the Earned Income Tax CreditAmerican Opportunity Tax Credit, the Child Tax Credit or the Credit for the Elderly.
  • Bilingual Help. Some VITA and TCE sites provide bilingual assistance.
  • Help for Military. Many military bases have VITA sites. These sites offer free tax assistance to members of the military and their families. Volunteers can help with military tax topics. Some of these include special rules and tax benefits that apply to those serving in combat zones.
  • Self-Preparation Option. At many VITA sites, people who earn $64,000 or less may be able to prepare their own tax returns. They can do this using free web-based software. This option is for those who do not have a home computer or do not need much help.

The FDA admitted “regulatory ambiguity” is hampering technology innovation in healthcare. The agency released three guidance docs last week in an effort to clarify. Developers have waited six years for one: FDA will generally not regulate “clinical decision support tools that allow the provider to independently review the basis for the recommendations.” But some feel the agency didn’t go far enough. Microalbuminuria is a term to describe a moderate increase in the level of urine albumin. It occurs when the kidney leaks small amounts of albumin into the urine, in other words, when there is an abnormally high permeability for albumin in the glomerulus of the kidney. Normally the kidneys filter albumin, so if albumin is found in the urine it’s then a marker of kidney disease. The term ‘microalbuminuria’ is now discouraged by KDIGO (Kidney Disease Improving Global Outcomes) and has been replaced by ‘moderately increased albuminuria’.

Associations/Indications

Microalbuminuria is an important adverse predictor of glycemic outcomes in pre-diabetes. Pre-diabetes individuals with increased microalbuminuria even in the so-called normal range is associated with increased progression to diabetes and decreased reversal to normoglycemia. Hence prediabetes individuals with microalbuminuria warrant more aggressive intervention to prevent diabetes in them.[2]

Causes/Prevention

Higher dietary intake of animal protein, animal fat, and cholesterol may increase risk for microalbuminuria,[3] and generally, diets higher in fruits, vegetables, and whole grains but lower in meat and sweets may be protective against kidney function decline.[4][5][6]

Diagnosis

The level of albumin protein produced by microalbuminuria can be detected by special albumin-specific urine dipsticks, which have a lower detection threshold than standard urine dipsticks. A microalbumin urine test determines the presence of the albumin in urine. In a properly functioning body, albumin is not normally present in urine because it is retained in the bloodstream by the kidneys.

Microalbuminuria can be diagnosed from a 24-hour urine collection (between 30–300 mg/24 hours) or, more commonly, from elevated concentration in a spot sample (20 to 200 mg/L). Both must be measured on at least two of three measurements over a two- to three-month period.[7]

An albumin level above the upper limit values is called “macroalbuminuria”, or sometimes just albuminuria. Sometimes, the upper limit value is given as one less (such as 300 being given as 299) to mark that the higher value (here 300) is defined as macroalbuminuria.[8]

To compensate for variations in urine concentration in spot-check samples, it is helpful to compare the amount of albumin in the sample against its concentration of creatinine. This is termed the albumin/creatinine ratio (ACR)[9] and microalbuminuria is defined as ACR ≥3.5 mg/mmol (female) or ≥2.5 mg/mmol (male),[10] or, with both substances measured by mass, as an ACR between 30 and 300 µg albumin/mg creatinine.[11] For the diagnosis of microalbuminuria, care must be taken when collecting sample for the urine ACR. An early morning sample is preferred. The patient should refrain from heavy exercises 24 hours before the test. A repeat test should be done 3 to 6 months after the first positive test for microalbuminuria. Lastly, the test is inaccurate in a person with too much or too little muscle mass. This is due to the variation in creatinine level which is produced by the muscle.[12]

Responding to Opioid Crisis, FDA Puts More Restrictions on Imodium

TUESDAY, Jan. 30, 2018 (HealthDay News) — Increasingly, people addicted to opioid painkillers are using dangerously high doses of the diarrhea drug Imodium (loperamide), either to get high or to help ease withdrawal.

So, on Tuesday the U.S. Food and Drug Administration said it’s putting new restrictions on the packaging of the medication, dubbed by some as “the poor man’s methadone.”

“When higher than recommended doses are taken we’ve received reports of serious heart problems and deaths with loperamide, particularly among people who are intentionally misusing or abusing high doses,” FDA commissioner Dr. Scott Gottlieb said in an agency news release.

Because opioid abusers are using the drug in greater numbers, the FDA is requesting that makers “change the way they label and package these drugs, to stem abuse and misuse,” Gottlieb said.

The FDA already slapped a warning on OTC loperamide labeling in the spring of 2017, cautioning users about the dangers of misuse.

The latest changes relate to the drugs’ packaging.

Specifically, packaging of Imodium should now only “contain a limited amount of loperamide appropriate for use for short-term [‘Traveler‘s’] diarrhea according to the product label,” Gottlieb said.

For example, that might mean a package would only contain eight 2-milligram capsules of the diarrhea drug in a blister pack, the FDA said.

The new rules will also seek to eliminate the sale of loperamide in large bottles — sales that typically occur via the Internet, Gottlieb said.

“The abuse of loperamide requires the purchase of extremely large quantities,” he noted, and “today’s action is intended to change how the product is packaged, to eliminate these large volume containers. We know that many of the bulk purchases of these large volumes are being made online through major online web retailers.”

One busy emergency room doctor said he’s witnessed the problem firsthand, and “applauds” the FDA’s latest move.

“I most often encounter patients who abuse loperamide either to get high or to self-treat symptoms of opioid withdrawal,” said Dr. Robert Glatter, of Lenox Hill Hospital in New York City. “Many people view it as low risk since they think it’s only medicine to treat diarrhea.”

“But those who abuse it to get high are gambling with their life, in essence,” he said. “Simply put, escalating use may place you at risk for an overdose. An overdose of loperamide can make you stop breathing, drop your blood pressure, or even cause a fatal heart arrhythmia.”

But Glatter said even more can be done to curb this new danger.

“It also starts with education, and informing parents and teens via social media platforms of the dangers of loperamide abuse,” he said.
